Getting started.
Prior to starting your new contract we'll need you to completed, sign and return a number of important documents to us.  Where possible, we have included the documents in your Mindworx Starter Pack along with some other information to make your transition as smooth as possible.  In order to use the interactive elements of your Starter Pack you will need the latest version of Adobe Reader, click here to download Adobe Reader 9.

For PAYG Contractors:
>  a signed contractual agreement
a completed contractor details form
a completed and signed tax file number declaration form
a completed and signed superannuation choice form
a signed information statement for job seekers

a completed and signed no claims declaration form
a copy of your workers' compensation certificate (if using own insurance)
a copy of your public & private liability insurance certificate (if using own insurance)
a copy of your professional indemnity insurance certificate (if using own insurance)

Getting paid.

To ensure you are paid on time, please submit your weekly timesheets via email to mw_admin@mindworx.com.au or fax to +61 7 3233 9099 by no later than 12noon every Monday (AEST)
